Instructions

Step 1:

Fill out the Criminal Background Check form and return it to the address listed above.  Please note that you must fill out the Basic Criminal Background Check form every three years and the Diocese of La Crosse Confidential Questionnaire annually.

* If you are a new hire, then you must also complete a Comprehensive Criminal Background Check which includes filling out the "Criminal Background Check Form" above, the "Confidential Questionnaire" below, and a Fingerprint Check.  Talk with your Office Manager regarding the Fingerprint Check.  If you have any further questions contact Jen Mickschl.

*Note:  All participants under the age of 18 will go through a safe environment training class that is appropriate to their age group.  For more information see your Priest, DRE, CRE, or Youth Minister.
