Step 1:

Fill out the Criminal Background Check form and return it to the address listed above.  Please note that you must fill out the Criminal Background Check form every three years and the Diocese of La Crosse Confidential Questionnaire annually.

Read all of the required reading.

           Watch the Safe Environment Video      

Step 4:

            Fill out the Receipt, Acknowledgement, and Agreement Form    

*Note:  All children and youth will go through a safe environment training class that is appropriate to their age group.  For more information see your Priest, DRE, CRE, or Youth Minister.
