Below are opportunities to download the common forms required of all Youth and Young Adult Ministry events. If you are unsure whether or not a form is necessary, please review the policies page or contact the office.

Common Forms

The following forms are required of all Diocesan events:

• Diocesan Medical Release Form - Child

• Diocesan Medical Release Form/Field Trip Waiver (3 pages) - Adult

• Safe Environment Training and Forms - Adult      *(Please note all Criminal Background Check Forms are to be sent to the Safe Environment office at the Diocese. The "Verification of Safe Environment Training Form is to be kept in your parish office)
